Pamela Lee Borzick,, 61 of Shadyside, Ohio died Saturday, March 20, 2021 at the Wheeling Hospital in Wheeling, West Virginia. She was born in Great Falls, Montana on July 4, 1959 daughter of the late Richard “Dick” and Patricia Mort Cook. Pam was an office clerk for West Virginia Choices in Washington Lands. 

In addition to her parents, she was preceded in death by her husband, Lawrence David Wallace on September 1, 2006 and daughter, Mystic E. Fields on July 1, 1991. Surviving are her husband, James E. Borzick, son, Jesse Fields of Neffs, Ohio, brother, Scott Cook and sister, Stacey Cook both of Shadyside, Ohio.

Pam has requested that there be no visitation or services. Arrangements by the Bauknecht-Altmeyer Funeral Homes & Crematory 441 37th Street Bellaire, Ohio.

I did not know Pamela personally but have known her husband Jim for over 40 years and to this day consider him a good friend. Condolences to all of her loved ones she left behind. Please know that she is out of pain and discomfort. Please try to go on my friend Jim, I truly believe that’s what she would have wanted for you. Be strong and have faith that the two of you will once again be reunited when it’s your turn to leave this life in Earth.

Many good memories of Pam at our work place. She loved nature. Loved joking with her, she made me laugh many a time. She was a good friend.
